DMHA is asking our membership for feedback on our Head Coaches again this year. Feedback and communication are critical components for coaches.
The purpose of this
form is to enable objective feedback regarding the current season of play. The feedback forms are reviewed independently by the President and Vice Presidents of the DMHA Executive. Feedback is then provided to the Coach Selection Committee prior to interviews for the forthcoming season.Your feedback helps to ensure we select from the best Head Coach applicants. Please keep your feedback honest and objective.
For U5-to-U13 age groups, PARENTS are to complete this
form; For U15 and above age groups PARENT OR PLAYERS may complete the
form.
