ATTENTION DMHA HOCKEY FAMILIES
The following information is important for all DMHA hockey activities for the 2021-2022 season.  Please read all of the following carefully. 

Local and provincial public health officials require that all DMHA participants (bench staff, players, volunteers, etc.), including all spectators of any age, MUST complete a COVID-19 screening each day before entering the Flight Exec Centre for any DMHA activity or event.  The screening must be completed no more than 24 hours before arrival. 

COVID-19 screening will be completed by using the following website https://dorchesterhockey.selfscreening.ca/ . If you arrive at the Flight Exec Centre without having completed the required screening, you will see DMHA COVID-19 Screening QR codes posted on entrance doors for your convenience.  Use the QR code to complete the required screening before entering the arena.  DMHA will monitor COVID-19 screening for compliance.

The Ontario Minor Hockey Association (OMHA) has implemented a policy that requires all affected persons to provide proof of full vaccination (a person is not fully vaccinated until 14 days period after receiving their second dose of an approved vaccine) by October 31, 2021 or earlier.  Accordingly, an affected person must have received their second dose by no later than two weeks before October 31, 2021.  The following are affected persons: 


• Players born in 2009 or earlier 
• Team Officials (Coaches, Trainers and Managers, etc.) 
• Game Officials (referees, linespersons, timekeepers, etc.) 
• Association Officials (minor hockey association directors, officers, executive and staff) 
• OMHA Officials (directors, officers, executive and staff, including hockey development personnel) 
• Volunteers  
• Instructors (third parties engaged by Associations or Teams to provide specialized instruction (e.g., power skating, goaltending coaching) 


Proof of vaccination for anyone outlined above who is participating in a DMHA activity MUST be uploaded to 
https://dorchesterhockey.selfscreening.ca/ prior
to October 31, 2021.  To upload proof of vaccination:

1.     Visit https://dorchesterhockey.selfscreening.ca

2.     Click on the Player Self-Screening Questionaire and scroll to the bottom

3.     Click on the Upload Covid Documents Here button

4.     Complete the form and upload related document

Beginning on Wednesday, September 22, 2021, anyone aged 12 years and older entering 
an indoor recreational facility must be fully vaccinated against COVID-19.  A security guard will be posted at the entrance of the Flight Exec Centre.  The security guard will be checking for proof of vaccination and will deny entrance to anyone who fails to present proof of vaccination or are unvaccinated. Anyone entering must present proof of their vaccination, as well as, a valid form of identification (driver’s license, passport or birth certificate).  Everyone will be expected to enter and exit through the South doors.  You can find the Municipality of Thames Centre COVID-19-Proof of Vaccination Policy linked here

Between September 22, 2021 and October 31, 2021, children between ages 12 and 18 will be allowed to enter the Flight Exec Centre without showing proof of vaccination but only “solely for the purpose of actively participating in an organized sport.”  This will also apply, during that period of time only, to anyone entering the Flight Exec Centre to coach, volunteer, referee, or perform work for DMHA.  These exceptions do NOT apply to spectators.  The OMHA policy above will apply effective October 31, 2021.  

Please note that parents of DMHA participants are still responsible for their children at all times while at the Flight Exec Centre.  Parents of younger DMHA participants must arrange appropriate supervision of their participants while attending for any DMHA activity, whether the parent is able to meet the entry requirements for the facility or not.
